Grants of up to $10,000 to help those 16-65 years with high physical care needs living at home and at risk of entering inappropriate housing, for equipment, home modifications and essential support unable to be funded through other means (e.g. NDIS). Individuals and eligible organisations on behalf of an individual can apply.
